Job Title: Social Services Director

We are seeking a highly skilled and compassionate Social Services Director to join our team at Skilled Nursing Facility.

Job Summary:

The Social Services Director will be responsible for identifying and addressing the social, emotional, and psychological needs of our residents. This includes ensuring the continuing development of each resident's full potential during their stay and assisting in the planning for their discharge.

Key Responsibilities:
  • Identify and participate in process improvement initiatives to enhance the customer experience and improve workflow.
  • Manage department staff, including hiring, training, and developing, coaching and counseling, and terminating as needed.
  • Provide timely and accurate completion of Social Services portion of the MDS, Resident Assessment Protocols, Resident Care Plan, and Progress Notes.
  • Assist residents and families with social, emotional, and family issues.
  • Participate in resident care planning by identifying social and emotional needs.
  • Maintain progress notes and make referrals to support agencies as needed.
  • Develop and implement discharge plans.
  • Serve as liaison for general information resources for families, residents, and the public.
  • Visit residents and perform needed services.
  • Coordinate logistics and content for monthly Family Council Meetings.
  • Coordinate resident room changes.
  • Work with Activity Coordinator to provide requested effects for residents.
  • Promote favorable working conditions and relationships with administration, medical staff, and other departments.
  • Work closely with the facility mental health provider to ensure resident psychosocial needs are identified and services are provided.
  • Assist in educating the community regarding aging, rights of residents, and facility services.
  • Orient residents to the long-term care environment and facilitate adjustment upon placement.
Requirements:
  • Bachelor's degree in social work or a human services field.
  • One year of supervised social work experience in a healthcare setting.
  • Effective verbal and written English communication skills.
  • Highest level of professionalism with the ability to maintain confidentiality.
  • Ability to communicate at all levels of organization and work well within a team environment.
  • Customer service oriented with the ability to work well under pressure.
  • Strong attention to detail and accuracy, excellent organizational skills, and ability to prioritize and coordinate multiple projects.
  • Strong analytical and problem-solving skills.
  • Ability to work with minimal supervision, take initiative, and make independent decisions.
  • Approachable, flexible, and adaptable to change.
