Job Summary

We are seeking a highly motivated and experienced Case Manager to join our team at the Brooklyn Public Library Adult Learning Program. The successful candidate will be responsible for providing student-centered adult literacy services to participants in the Literacy Zone Welcome Center at the Eastern Parkway Library.

Key Responsibilities
  • Conduct comprehensive assessments to develop plans of service for participants, providing intensive one-on-one and/or small group services to meet their educational and career goals.
  • Plan and coordinate community events to promote program services and meet program targets.
  • Identify post-secondary transition programs and assist participants in becoming familiar with the American workforce, improving their English skills, and succeeding in post-secondary education or training.
  • Assist with program outreach, recruitment, information sessions, assessments, orientations, and goal setting for enrolled participants.
  • Communicate effectively with key staff and instructors to identify barriers preventing participants from meeting their educational needs.
  • Collaborate with community partners to provide wrap-around services, including health literacy, financial literacy, and more.
  • Research and assist participants in acquiring related supportive services, such as computer literacy, transportation, public assistance, and childcare.
  • Facilitate and/or coordinate various workshops and information sessions.
  • Attend community partner meetings and bi-monthly case management team meetings.
  • Maintain accurate case notes using the ASISTS database.
  • Monitor participants' progress and follow up frequently.
Requirements
  • Some college required; Bachelor's degree in education or a related field preferred.
  • Relevant experience in adult education (HSE) and counseling individuals or facilitating informational sessions.
  • Strong awareness of barriers facing adult learners and good knowledge of available educational, training, and employment resources.
  • Good knowledge of labor market trends and requirements.
  • Initiative, resourcefulness, tact, and courtesy.
  • Proficient in MS Word, Excel, Google Drive, Outlook, and Social Media.
  • Able to travel to various program sites and work alternate Saturdays, evenings, and day shifts.
