About the Role
This position involves coordinating care for patients with complex pain conditions. You will work closely with multidisciplinary teams to develop and implement individualized care plans that address patients' unique needs. Your expertise will help ensure seamless transitions between treatments, appointments, and hospital stays.

Key Responsibilities
- Coordinate patient care services to optimize treatment outcomes
- Collaborate with healthcare professionals to develop and implement comprehensive care plans
- Communicate effectively with patients, families, and caregivers to ensure informed decision-making
- Monitor and report on patient progress to facilitate timely interventions
- Participate in quality improvement initiatives to enhance patient satisfaction and outcomes

Requirements
To be successful in this role, you should have a current Registered Nurse (RN) licensure in the state of SD or multi-state privilege to practice in SD if residing in a compact state outside of SD. Previous experience in medical, surgical, or pediatric nursing is beneficial, but not required. Basic Life Support certification is mandatory within six months of employment.
