About the Role

We are seeking a compassionate and skilled Nurse Practitioner (NP) to join our team at Abode Care Partners. The ideal candidate will have experience in providing personalized medical care to seniors and patients with complex needs.

The role involves performing evidence-based medical assessments, collaborating with healthcare professionals, and managing patient records. Strong communication skills and a patient-centered approach are essential.

Key Responsibilities
  • Conduct comprehensive medical assessments to identify health issues and develop individualized treatment plans
  • Collaborate with caregivers and other healthcare professionals to ensure seamless care coordination
  • Manage patient records accurately and maintain confidentiality
  • Provide referrals to specialists as needed
Requirements
  • Active state NP license and current certification
  • Master's degree in Nursing (MSN)
  • At least two years of experience in nurse practitioner roles
  • Strong interpersonal communication skills
  • Excellent time management skills
