Job Summary

We are seeking a skilled RN Case Manager to join our team at North Shore Medical Center. As a key member of our interdisciplinary healthcare team, you will be responsible for assessing patient care needs and monitoring utilization of resources to ensure timely and appropriate payment from third parties.

Key Responsibilities:

  • Review and ensure appropriate utilization of resources, alerting physicians regarding cases not meeting admission criteria, with questionable medical management, or with documentation problems.
  • Facilitate the plan of care through the continuum from entry to discharge, collaborating with others, and managing various health interventions.
  • Capture, document, and maintain current information dealing with hospital standards, the plan of care, progress, and information substantiating the need for continued stay.
  • Identify and act on variances from standards of care, assisting the team with identifying and resolving quality problems related to scheduling, duplicate ordering of tests/services, and significant variances from care paths.

Requirements:

  • Graduate of a School of Nursing required
  • BSN preferred
  • Licensed as an RN in the State of Florida
  • Previous medical or psychiatric experience preferred
