Healthcare Navigator

We are seeking a highly skilled and compassionate Healthcare Navigator to join our team at PruittHealth. As a key member of our team, you will be responsible for navigating complex healthcare systems to ensure seamless care delivery to our patients.

• Screens appropriate referrals and coordinates admission processes to ensure timely access to care.
• Coordinates services with providers, billing reconciliation, and outreach to community referral sources.
• Presents new admits, annual assessments, hospitalizations, Home Health admits, and discharges in team meetings with Medical Directors.

Additional responsibilities include:

• Completing all new admission paperwork for signed forms and in-home assessments.
• Participating in quarterly home visits and PCP visits with all requirements completed.
• Tracking hospital/ER visits by completing paperwork, requesting records, and communicating with hospitals.
• Coordinating with outpatient therapy centers, home health agencies, and rehab facilities to assess member needs.
• Attending trainings as required and training new hires as needed.

This role requires strong communication and organizational skills, with the ability to navigate complex healthcare systems. We offer a competitive salary of $62,000 - $75,000 per year, based on experience.
