We are seeking a highly skilled Clinical Pharmacist to join our team at HopeHealth, a leading not-for-profit provider of home health, palliative and hospice care in Massachusetts and Rhode Island.

Job Description

The successful candidate will provide clinical pharmacy services to patients with acute, life-threatening illness, advanced illness, long-term care or terminal illness. Key responsibilities include:

  • Providing pharmacy consultations to staff to optimize drug therapies in areas such as common hospice symptom management within the formulary.
  • Collaborating with the medical and clinical teams to ensure high-quality, cost-effective pharmaceutical care.
  • Facilitating the identification of medication use issues including allergies, drug-drug interactions and duplication of therapy.

Requirements and Qualifications

To be considered for this role, candidates must have:

  • A Doctor of Pharmacy (PharmD) degree from an accredited institution.
  • Licensure and registration as a pharmacist in Massachusetts and Rhode Island.
  • At least two years of clinical practice experience in pharmaceutical care for patients with acute, life-threatening illness, advanced illness, long-term care or terminal illness.
  • Excellent verbal and written communication skills with individuals and groups.
