Job Title: Physical Therapy Assistant Specialist
">About the Job: We are seeking a skilled Physical Therapy Assistant Specialist to join our team at All Medical Personnel. As a Physical Therapy Assistant Specialist, you will play a critical role in providing high-quality physical therapy services to patients in their homes.

- Collaborate with the interdisciplinary team, patients, and families to develop holistic plans of care focused on achieving physical therapy goals.">
- Instruct, coach, and guide patients and families to achieve optimal health and well-being outcomes while remaining in their communities.">
- Promote optimal health, well-being, and quality of life for home health patients and their families by providing skilled, intermittent physical therapy in the patient's home.">
- Practice independently in the patient's home using evidence-based practices for assessment, clinical decision-making, clinical practice, and coordination of care across the continuum.">
- Act as case manager and team lead for patients requiring home health therapy services when nursing is not involved or physical therapy is the primary patient need.">
- Assess patients and collaborate with the interdisciplinary team and primary care providers to develop patient-centered plans of care.">
- Provide physical therapy treatment to patients and families as ordered.">
- Complete accurate clinical documentation to support services provided within specified timeframes.">
- Provide physical therapy to pediatric, adolescent, adult, and geriatric patients based on principles of growth and development and life stages.">
- Maintain communication regarding patients with the case manager and other disciplines via care conferences or other appropriate methods.">
- Report acute medical problems and condition changes to primary care providers.">
- Supervise Physical Therapy Assistants, Home Health Aides, and students in the clinical setting.">
- Complete regulatory required assessments within specified timeframes.">
- Complete medication reconciliation at specified timeframes.">
Requirements:
">- ">
- 1-2 years of experience in an outpatient rehabilitation setting.">
- At least two (2) years of physical therapy experience highly preferred.">
- One (1) year of experience in home health or other health settings with a geriatric emphasis.">
- Current license in Physical Therapy or ability to obtain prior to hire.">
- American Heart Association Healthcare Provider BLS preferred upon hire, and required within 90 days of hire.">
- Current Driver's license.">
- Excellent customer service skills.">
- Ability to work without direct supervision.">
- Caregiver responsibilities must be carried out with sensitivity, respect, and compassion.">
- Demonstrate the highest level of professionalism when interacting with patients, families, physicians, and hospital staff members.">
- Able to maintain effective relationships with other healthcare professionals.">
- Able to develop skills in the use of technology used in home care settings.">
Salary Range: $55,000 - $70,000 per year, depending on experience.
