Key Responsibilities:
  • Work closely with medical providers and the Care Coordination team to provide comprehensive care services.
  • Conduct screenings and assessments to identify patients in need of substance use recovery services.
  • Develop and implement individualized care plans to address the unique needs of each patient.
  • Provide healthcare education and complete case management activities to support patient recovery.
  • Facilitate therapeutic or educational groups to promote patient engagement and support.
Requirements:
  • CACD, CRADC, MAAT, or MISA certification from IAODAPCA.
  • Two years of experience working with individuals with substance use disorders, completing screenings and assessments.
  • Additional experience providing healthcare education and completing case management activities.
  • One year of experience facilitating therapeutic or educational groups.
  • High School diploma or GED.
  • Experience working with culturally diverse populations.
  • Valid driver's license and ability to have driving privileges through the agency's insurance program.
