Care Manager II Job Description

At Institute on Aging, we are committed to providing innovative healthcare models that transform lives and enhance communities. As a Care Manager II, you will play a vital role in shaping the future of aging in place.

Key Responsibilities:

  • Conduct comprehensive assessments of clients with multiple medical and psychosocial needs.
  • Develop and implement personalized care plans that ensure quality care and support.
  • Collaborate with healthcare professionals and community providers to coordinate services and interventions.
  • Provide crisis intervention, advocacy, and problem-solving support to clients and their families.
  • Document all case management activities and maintain accurate records.

Requirements:

  • Master's degree in social work or a related field.
  • One year of experience working with disabled adults and/or older adults.
  • Exceptional communication and presentation skills.
  • Case management skills and experience in the community healthcare delivery system.

Compensation:

$75,000-$86,000 annual range. This amount is not necessarily reflective of actual compensation, which is dependent on experience, education, and other factors.
