Job Responsibilities:

We are seeking an experienced Occupational Therapist to join our team in a Skilled Nursing Facility (SNF) setting. In this role, you will assess patients' physical and mental abilities to determine rehabilitation needs, focusing on activities of daily living (ADLs) and functional mobility. You will develop and implement individualized treatment plans to help patients regain or improve their independence in daily activities.

You will provide therapeutic interventions to enhance fine motor skills, coordination, strength, and mobility, using a variety of techniques and equipment. You will also educate patients and their families on adaptive techniques and the use of assistive devices to enhance safety and independence at home.

As part of our multidisciplinary healthcare team, you will collaborate with physical therapists, speech therapists, and nursing staff to ensure comprehensive patient care. You will monitor and document patient progress, adjusting treatment plans as necessary to achieve optimal outcomes.

You will also participate in regular team meetings and contribute to the development of care plans and goal setting for patients. Engaging in professional development activities to stay current with best practices, regulatory requirements, and advances in occupational therapy is also expected.

Job Requirements:

You will need a strong understanding of occupational therapy principles, especially in the context of a Skilled Nursing Facility (SNF) environment. Excellent communication and interpersonal skills are required for interacting with patients, families, and a multidisciplinary team.

You will also need proficiency in creating and implementing individualized treatment plans based on patient needs. An active license in good standing as an Occupational Therapist (OT) is required, as well as a minimum of 2 years of experience in a similar role. Certification in Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) is also necessary.
