Emergency Shelter Case Manager

About the Role: We are seeking a skilled and compassionate Case Manager to join our team at Metropolitan Ministries. As a Case Manager, you will provide critical support to individuals and families experiencing homelessness, helping them navigate the complexities of our emergency shelter programs.
Key Responsibilities:
Provide case management services to emergency shelter families, developing and implementing personalized plans to address their unique needs.
Coordinate with community service providers and program staff to ensure seamless support and referrals.
Maintain accurate and confidential client records, utilizing best practices to ensure data integrity.
Participate in weekly staff and group meetings, contributing to a collaborative and supportive team environment.
Requirements:
A minimum of a Bachelor's degree in a human services field and two years of relevant work experience.
Strong interpersonal and communication skills, with the ability to work effectively with diverse populations.
Bilingual (Spanish) preferred, but not required.
Ability to work independently and as part of a team, with a high degree of maturity and sensitivity to cultural and individual differences.
