Job Summary

We are seeking an experienced Registered Nurse to join our Transitional Care Unit team at Fort Sanders Regional Medical Center. As a Geriatric Care Nurse Specialist, you will play a critical role in providing exceptional care to patients transitioning from hospital to home.

Key Responsibilities:
  • Completes nursing history and initiates patient education on disease management and self-care.
  • Collaborates with interdisciplinary team members to develop and implement individualized care plans.
  • Provides direct patient care, including administering medications, performing treatments, and monitoring vital signs.
  • Supervises licensed practical nurses, certified nursing assistants, and other support staff to ensure seamless care delivery.
  • Participates in quality improvement initiatives and contributes to unit-based committees to enhance patient outcomes and satisfaction.
Requirements:
  • RN license in good standing with the Tennessee Board of Nursing.
  • One year of experience as a registered nurse, preferably in a transitional care or geriatric setting.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and interprofessional teams.
What We Offer
