We are seeking a skilled and compassionate Home Care Coordinator Specialist to join our team at Kintegra Health. As a vital member of our healthcare team, you will play a key role in ensuring the highest quality of care for our patients.
About the Role:
- Job Summary: The Home Care Coordinator Specialist will be responsible for assessing the home care needs of our patients, identifying and developing specific plans of care, and coordinating 24-hour care delivery and implementation of all home care services.
- Key Responsibilities:
- Assess patient needs: Conduct initial and periodic assessments to identify patient needs and develop specific plans of care.
- Coordinate care delivery: Coordinate 24-hour care delivery and implementation of all home care services, including personal care services and management of home supplies.
- Manage care logistics: Reconcile invoices for personal care service hours and home supplies, coordinate and authorize Durable Medical Equipment (DME), and manage home supplies such as incontinence, diabetic, colostomy, nutritional, and other supplies.
- Support interdisciplinary team: Support the Interdisciplinary Team and promote unity among team members while interacting with the team, other co-workers, and/or participants.
- Communicate effectively: Communicate and work efficiently with the IDT when evaluating potential enrollees, participate in participant care planning, and enter all care plan information in a timely manner as per organizational protocols.
Requirements:
- Bachelor's degree or equivalent experience.
- Minimum of 1 year's experience working with the frail elderly population preferred.
- Current, valid NC driver's license and vehicle.
- Must be able to treat geriatric patients.
- Immunizations: Be medically cleared for communicable diseases and have all immunizations up to date before engaging in direct participant contact.
Salary: $60,000 - $70,000 per annum.
