We are seeking a skilled and compassionate Physical Therapist to join our team at JAG Physical Therapy. At JAG Physical Therapy, we prioritize our patients' needs and offer a comprehensive benefits package to support their well-being. Our compensation package includes a competitive salary, up to $2,500 relocation bonus, and $7,500 sign-on bonus. In this role, you will work closely with interdisciplinary teams to assess patients' strengths and weaknesses, develop tailored treatment plans, and implement effective therapeutic interventions. You will also have opportunities to grow professionally through our New Grad Mentorship Program and Professional Development Growth Tracks.
Key responsibilities include:
- Collecting and recording patient care data to ensure accurate billing and productivity.
- Preparing for evaluations by obtaining patient history, researching unfamiliar diagnoses, and selecting and administering tests and measures.
- Formulating diagnoses, goals, prognosis, and plans of care based on evaluation results and best practice.
- Establishing treatment plans, collaborating with interdisciplinary teams, and assessing the need for modifications to plans of care and goals.
- Providing education and training to patients and their support systems, including ADLs/IADLs, therapeutic activities, rehabilitation equipment prescription, and community reintegration.
To succeed in this role, you must possess a Bachelor's or Master's Degree in Physical Therapy, be licensed in the state practicing in, and maintain current CEU requirements. Experience with EMR software is preferred, as is basic computer skills and strong clinical decision-making abilities.
