Patient Transition Coordinator

The Compassus Patient Transition Coordinator is responsible for modeling the company's values and promoting the Compassus philosophy. This role is key to care transitions, supporting referrals to hospice, home health, home infusion, and palliative care.
Key Responsibilities- Organize and track medical records for patient transitions to home health providers.
- Identify post-acute care needs and collaborate with the Care Transition Coordinator.
- Collaborate with Compassus/Ascension at Home intake teams.
- Complete the transition of care, including submission of complete referrals and securing outside agency placement if necessary.
- Organize and prioritize large volumes of information.
- Display exceptional customer service to patients, families, and referral sources.
- Handle confidential and non-routine information.
- Manage and create general correspondences, memos, charts, tables, graphs, business plans, etc.
- Bachelor's degree in a health-related field preferred.
- Minimum of one year of experience in a related healthcare setting preferred.
- One year of experience in home health, hospice, and/or palliative care preferred.
- Prior experience with electronic medical records preferred.
- Knowledge of home health, hospice, and palliative eligibility requirements preferred.
