Key Responsibilities:
- Provide professional nursing care to patients in their homes, including assessment, planning, implementation, and evaluation of care plans.
- Collaborate with patients, families, and healthcare teams to develop and implement individualized care plans.
- Conduct comprehensive assessments of patients' physical, social, and emotional needs, and develop plans to address these needs.
- Provide education and support to patients and families to promote health and well-being.
- Document patient care and progress in a timely and accurate manner.
Requirements:
- Valid New York State license and current registration to practice as a Registered Professional Nurse.
- Basic Life Support (BLS) certification through the American Heart Association.
- Ability to travel regularly to patient assignments across New York City via public transportation and/or private vehicle.
- Demonstrated commitment to continued professional development.
