Clinical Anticoagulation Management Specialists play a vital role in the healthcare team, working closely with patients to manage their anticoagulation therapy.
About This Role
- This position is responsible for monitoring medication therapy and performing ongoing assessments of patient risk factors and therapeutic regimens.
- In collaboration with physicians and other healthcare professionals, this specialist will interpret medical records and laboratory results to adjust medication therapy according to established guidelines and standards.
Responsibilities
- Provide high-quality care to patients by monitoring medication therapy and coordinating follow-up appointments as needed.
- Collaborate with multidisciplinary teams to develop and implement individualized treatment plans that meet each patient's unique needs.
- Stay up-to-date on the latest research and best practices in anticoagulation management through ongoing education and training.
Requirements
- Active Registered Nurse (RN) or Advanced Registered Nurse Practitioner (ARNP) licensure with prescriptive authority in Washington state is required.
- A minimum of three years of experience in clinical settings, preferably in anticoagulation management.
