About the Role:

This exciting opportunity allows you to make a meaningful difference in the lives of patients and families by providing exceptional care and support. As a Patient Care Nurse Specialist, you will be responsible for delivering high-quality patient care services in an ambulatory setting, collaborating with interdisciplinary teams to develop and implement comprehensive care plans.

Responsibilities:

  • Assessment: Collect data and complete nursing assessments utilizing area-specific standards and tools, recognizing subtle changes in patients' conditions and responding appropriately.
  • Planning/Outcomes: Develop and implement individualized care plans, making referrals as needed, and ensuring timely communication with patients, families, and healthcare teams.
  • Implementation: Provide direct care to patients in a safe, timely, and cost-effective manner, consistent with established care plans and priorities, while maintaining a focus on quality and safety.
  • Evaluation: Monitor and document patient progress, notifying providers of any significant changes or concerns.

Requirements:

  • Education: Associate's Degree in Nursing; BSN preferred.
  • Licensure: Licensed to practice as a Registered Nurse in South Carolina.
  • Work Schedule: Monday – Friday, No Weekends, No Call.

Estimated Salary: $65,000 - $85,000 per year, depending on experience and qualifications.
