About the Role

VNS Health is seeking a skilled Substance Use Counselor to join our team in Brooklyn, New York. As a key member of our Behavioral Health team, you will play a vital role in providing compassionate care and support to individuals struggling with substance use challenges.

Key Responsibilities
  • Conduct phone and field outreach to locate and enroll clients in programs and services
  • Build relationships and trust with clients and their families, assisting with interactions with professionals on the team
  • Work collaboratively with an interdisciplinary team to discuss care needs and identify solutions to support clients
  • Apply shared experiences to build relationships and trust with clients
  • Educate clients about program services, benefits, and self-help techniques
  • Advocate for effective recovery-based services on behalf of clients
  • Teach and model symptom management and coping skills for resilience
  • Review service plans with clients and their families, providing ongoing education, guidance, support, and encouragement
  • Develop inventory of resources to meet client needs
  • Provide navigation services to help clients connect with community-based services and supports
  • Document in EMR in accordance with program policies and procedures
Requirements
  • High school diploma or equivalent required
  • Minimum of one year experience in a mental health, substance use treatment program, healthcare, or human services setting
  • Experience working with severely mentally ill, psychological trauma, and/or substance using populations preferred
  • Effective oral, written, and interpersonal communication and relationship building skills required
  • Ability to work independently and collaboratively on an interdisciplinary team
  • Computer literacy (electronic health records, word processing, email, internet research, data entry) required
  • Valid New York State driver's license, as determined by operational/regional needs
  • Bilingual skills in English and Spanish preferred
