We are seeking a highly skilled Family Nurse Practitioner to join our team at the Bronx Community Health Network (BCHN) in White Plains Road, Bronx.

Bronx Community Health Network (BCHN) was founded in 1996 with the mission to ensure that the socio-economically depressed and medically underserved residents of the Bronx have access to high-quality and comprehensive healthcare services.
We are a not-for-profit, community-based organization and Federally Qualified Health Center providing a wide range of comprehensive, coordinated primary medical, oral, and mental health care and related diagnostic, pharmacy, social support, and enabling services for medically underserved/uninsured residents.
About the Role- Provide high-quality care to adult and pediatric patients and their families.
- Work closely with a team of professionals to develop a comprehensive patient-centered plan of care to achieve expected outcomes.
Responsibilities:
- Deliver primary medical care to patients and manage acute and chronic conditions.
- Conduct age-appropriate assessments of patients, including history taking and physical examination in accordance with current standards of professional practice, regulatory requirements, and agency policies and procedures.
- Assess data collection in an ongoing systematic manner, focusing on physiological, psychosocial, cultural, spiritual, and cognitive status, patient/family education, family involvement, and patient advocacy.
- Work continually to ensure the delivery of quality clinical care in a safe, efficient, competent, and effective manner.
- Refer to other clinicians as clinically appropriate.
- Prepare records and reports as required in support of services rendered in accordance with established procedures.
- Maintain accurate and detailed medical records about each patient, including observations during examinations (via EMR).
- Administer or prescribe treatments and medications.
Qualifications:
- Master's Degree graduate of an accredited Nurse Practitioner program.
- New York State Licensed Certification and Licensure as a Nurse Practitioner.
Required Skills and Qualifications:
- Current certification in Basic Life Support (BLS) through the American Heart Association (AHA) or willing to obtain within a reasonable timeframe.
- Board Certified.
- Current active DEA license.
Preferred Qualifications:
- Relevant experience working in community-based healthcare settings.
- Experience/understanding of Electronic Medical Records (EMRs).
- The ability to demonstrate teamwork and effective interpersonal relations skills.
- Excellent written/oral communication and computer skills.
- Ability to foster and maintain a positive working relationship in a multidisciplinary team and clients.
Compensation:
$125,000 - $140,000 annually based on experience.
