About the Job

We are seeking a skilled Nurse Practitioner or Physician Assistant to join our team at Matrix Medical Network. This is a PRN (as-needed) position, with flexible scheduling and opportunities to work in various settings, including home visits, skilled facilities, and telehealth.

The ideal candidate will have a strong background in healthcare, excellent communication skills, and the ability to work independently in a fast-paced environment. As a member of our team, you will have the opportunity to make a positive impact on patients' lives, providing comprehensive health assessments and developing personalized care plans.

Key Responsibilities
  • Conduct thorough health assessments, including medical history, physical examination, and review of medications and social environment.
  • Develop and implement individualized care plans, taking into account patients' unique needs and goals.
  • Collaborate with primary care physicians, case managers, and other healthcare professionals to ensure seamless care coordination.
  • Provide education and support to patients and their families, promoting healthy behaviors and self-management techniques.
  • Maintain accurate and detailed records of patient interactions, assessment results, and treatment plans.
Requirements and Qualifications
  • Master's degree in nursing or physician assistant studies required.
  • Current licensure as a Nurse Practitioner or Physician Assistant in the state of practice.
  • Board certification in a relevant specialty.
  • A minimum of 1 year experience as a Nurse Practitioner or Physician Assistant.
  • Strong computer skills and familiarity with electronic medical records software.
  • Excellent verbal and written communication skills, with the ability to interact effectively with patients, families, and healthcare teams.
