Job Description:

We are seeking a highly skilled and experienced Registered Nurse to join our team as a Case Manager. The successful candidate will be responsible for conducting face-to-face needs assessments with our members, developing and implementing care plans, and coordinating services to meet their complex health and social service needs.

Responsibilities:
  • Travel to members' homes, nursing facilities, and other community-based settings to conduct needs assessments
  • Develop and implement care plans that address members' physical, behavioral, long-term services and supports (LTSS), social, and psychosocial needs
  • Coordinate care across the continuum of services, including referrals to special programs such as Disease/Chronic Condition Management, Behavioral Health, and Complex Case Management
  • Educate members and caregivers regarding health care needs, available benefits, resources, and services
  • Assist members in developing, implementing, and amending backup plans for gaps in provider coverage
  • Ensure approved support services are being provided as outlined in the plan of care
Requirements:
  • Current Registered Nurse licensure in the state of Delaware
  • 5+ years of Case Management and Discharge Planning experience, including experience discharging members from a facility setting
  • Experience completing assessments, developing service plans, and care plans
  • Collaboration with PCPs, Occupational Therapists, Behavioral Health, and Providers
  • Reliable transportation and ability to travel throughout the state
Preferred Qualifications:
  • Certified Case Manager (CCM)
  • Licensed Bachelor's Social Worker (LBSW)
  • Licensed Master's Social Worker (LMSW)
  • Licensed Clinical Social Worker (LCSW)
