Job Description: Nurse Practitioner - In-Home Care

This role offers a unique opportunity to redefine your work-life balance and make a meaningful impact in the lives of our members. As a Nurse Practitioner at EasyHealth, you will be responsible for delivering comprehensive assessments, including Annual Wellness & Quality Gap Care visits, in the homes of our members.

You'll have the chance to develop strong relationships with your patients and provide personalized care that addresses their unique needs. Your expertise and compassion will play a vital role in helping our members achieve better health outcomes.

Key Responsibilities:

  • Perform comprehensive assessments and screenings in an in-home setting
  • Accurately document clinical findings and code relevant information
  • Develop and implement personalized care plans tailored to each patient's needs
  • Identify and refer patients for routine, urgent, or emergent care management
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination
Requirements and Qualifications

To be successful in this role, you should possess the following qualifications and skills:

  • Current, unrestricted license as a Nurse Practitioner in the state(s) where services are rendered
  • Board-certified in geriatric or adult medicine specialty
  • 2+ years of hands-on experience as a Nurse Practitioner
  • Familiarity with HCC models, quality HEDIS measures, and STAR performance metrics
  • Effective communication and interpersonal skills
  • Proficiency in using computer, tablet, and mobile technology

  • Estimated annual salary: $120 per visit, up to 40 hours/week (based on 6-8 patients/day, 45min-1hr sessions)
