Job Summary

Colorado Permanente Medical Group is seeking a board-certified geriatric trained physician to join our team as a Comprehensive Care Specialist. This is an exciting opportunity to provide high-quality, patient-centered care to adult patients with complex medical, social, and behavioral needs.

Key Responsibilities
  • Provide comprehensive medical services to patients in a Primary Care setting, including diagnosis and treatment of internal disorders, such as hypertension, heart disease, and diabetes.
  • Analyze records, reports, and test results to diagnose medical conditions and develop treatment plans.
  • Prescribe or administer medication, therapy, and other specialized medical care to treat or prevent illness, disease, or injury.
  • Manage and treat common health problems, such as infections, influenza, and pneumonia, as well as serious, chronic, and complex illnesses in adults and the elderly.
  • Monitor patients' conditions and progress, and reevaluate treatments as necessary.
  • Collect, record, and maintain patient information, including medical history, reports, and examination results.
  • Make diagnoses when different illnesses occur together or in situations where the diagnosis may be obscure.
  • Explain procedures and discuss test results or prescribed treatments with patients.
  • Advise patients and community members concerning diet, activity, hygiene, and disease prevention.
  • Utilize computer systems to access patient records and other clinical information, including accurate coding.
  • Perform office procedures commensurate with scope of practice, such as minor office surgery, gynecology, and office orthopedics.
  • Refer patients to medical or surgical specialties for consultant services when necessary for patient well-being.
  • Work as part of a comprehensive care team that involves MA/LPNs, RNs, social work, and care coordinators.
Requirements
  • Board certification in Geriatric Medicine or a related field.
  • Completion of a Geriatric Fellowship or similar training.
  • Experience in complex care, geriatrics, and team-based approaches.
  • Ability to work in a fast-paced environment and prioritize multiple tasks.
  • Excellent communication and interpersonal skills.
  • Ability to work effectively in a team environment.
