About the Role

This part-time position involves coordinating care and services for selected patient populations through the acute care episode, across the continuum. You will work collaboratively with inter-disciplinary teams, both internal and external to the organization, to improve patient care through effective utilization and monitoring of healthcare resources.

Key Responsibilities
  • Coordinate discharge planning and utilization activities, providing oversight for the Case Management Associate as needed
  • Assess patients' goals and biophysical, psychosocial, environmental, economic/financial, and discharge planning needs
  • Procure services and resources for identified patients and families, serving as an advocate to promote achievement of agreed-upon goals
About You

To be successful in this role, you should possess excellent communication and interpersonal skills, with the ability to work effectively with patients, families, physicians, and other healthcare professionals. A Bachelor's Degree in Nursing or related field is required, with 3 years of registered nursing experience in an acute care hospital.
