Job Summary

We are seeking a skilled Physical Therapist to join our team, providing physical therapy services to patients in their homes. As a Physical Therapist with HarmonyCares, you will have the opportunity to make a meaningful difference in the lives of our patients, while enjoying a rewarding and challenging career.

Responsibilities
  • Provide comprehensive physical therapy services, including assessment, evaluation, and treatment planning.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
  • Supervise and instruct non-professional home health aide staff in the provision of personal care to patients.
  • Update care plans regularly to reflect changes in patient needs and progress.
  • Submit completed visit notes and participate in staff meetings and educational in-services as required.
Requirements
  • Current, unencumbered State Physical Therapy License.
  • One year of experience as a home care professional Physical Therapist, with competency in performing comprehensive assessments and OASIS-C.
  • Ability to make sound professional clinical judgment and assess patient needs.
  • Proficiency in clinical skills and Microsoft Office.
