Job Title: Clinical Pharmacist

We are seeking a highly skilled Clinical Pharmacist to join our team at Soliant Health. As a Clinical Pharmacist, you will play a critical role in providing exceptional patient care and contributing to the development of our pharmacy department.

Key Responsibilities:
  • Documenting and Reporting: Accurately document drug information questions, drug utilization evaluation activities, and adverse drug reactions.
  • Patient Care: Understand and respond effectively to the unique needs of patients across various age groups, as demonstrated by effective documentation in progress notes and pharmacy care plans.
  • Leadership and Guidance: Ensure adherence to departmental policies and procedures, as well as State and Federal laws and regulations, by pharmacy interns, residents, and technicians.
  • Formulary Management: Promote adherence to the approved Drug Formulary by the Pharmacy and Therapeutics Committee of the Medical Staff.
  • Charging and Billing: Ensure proper charging documentation is completed correctly for services provided.
  • Community Outreach: Promote and market the pharmacy department to the community, patients, hospital employees, and medical staff in an informal manner.
  • Communication: Maintain a positive, direct, and open communication line with members of the Medical Staff and their employees.
  • Clinical Competency: Maintain clinical competency through successful completion of self-study modules and providing clinical services in practice.
  • Teaching and Supervision: Provide teaching, direction, and supervision of pharmacy students, residents, and medical assistants working under your direction.
  • Professional Development: Participate in and support professional organizations.
  • Program Development: Take an active, participating role in the development and implementation of new programs, while supporting and facilitating existing programs.
  • Competency Assessments: Complete assigned competency assessments.
  • Additional Responsibilities: Complete additional responsibilities/projects when assigned by the Director.

Preferred Education: Completion of an advanced degree is preferred, but not required. Completion of an ASHP Accredited Residency and/or Fellowship is also preferred, but not required.

Required Experience: Demonstrated experience in anticoagulation therapy, bridge therapy, and drug information is required.
