Scope of Work

We are seeking a compassionate and skilled End-of-Life Care Specialist to join our interdisciplinary team. The successful candidate will assess, plan, implement, coordinate, monitor, and evaluate services needed for patients and families seeking end-of-life care.

Job Description

The End-of-Life Care Specialist will create an outcome-based care plan that adapts to the evolving needs of patients and families, ensuring a peaceful and symptom-free transition. This role specializes in providing comfort and symptom management to individuals with a life expectancy of six months or less, offering care in various settings, including homes and nursing facilities.

Essential Functions:
  • Communicate effectively with patients, families, and team members; develop and maintain personalized patient care plans.
  • Administer medications following regulatory guidelines and inform care coordinators of significant patient changes.
  • Advocate for patients and families while ensuring continuity of care with the entire healthcare team.
  • Assess and address the educational needs of patients and their families, implementing suitable teaching strategies.
  • Monitor and report on patient conditions, initiating necessary interventions as required.
  • Guide and supervise the care provided by CNAs, LPNs, or other nursing staff according to guidelines.
  • Implement and evaluate a comprehensive care plan, adjusting it as necessary, and facilitate transitions as needed.
Qualifications:
  • Associate's Degree in Nursing - Required.
  • Bachelor's Degree in Nursing - Preferred.
  • 1 year of experience in acute care, emergency, or rehabilitation settings - Preferred.
  • Registered Nurse (RN) license in Michigan required upon hire.
  • Valid Driver's License in Michigan required upon hire.
Compensation and Benefits

This full-time position offers a competitive salary range of $65,000-$80,000 per annum, depending on experience. Our comprehensive benefits package includes:
