Job Summary:

We are seeking a skilled LPN to join our Hospice Team in Indianapolis. The successful candidate will provide skilled nursing care to patients in their homes, under the supervision of a registered nurse. The ideal candidate will have a strong background in nursing, excellent communication skills, and the ability to work independently.

Key Responsibilities:

- Provide skilled nursing care to patients in their homes, under the supervision of a registered nurse.
- Conduct thorough assessments of patients' physical, psycho-social, and environmental needs.
- Develop and implement individualized care plans in conjunction with the RN and interdisciplinary team.
- Provide effective communication to patients, families, and healthcare professionals.

Requirements:

- Graduate of an accredited school of vocational nursing.
- One year experience as a Licensed Vocational Nurse in a clinical care setting required. Home health experience preferred.
- Good interpersonal skills and ability to work independently.
