East Bay Community Action Program (EBCAP) is seeking a Registered Nurse Care Manager to provide ongoing health assessments and care management to adults in our Behavioral Health Services Division.
We are looking for a highly professional and motivated individual who enjoys working with a multi-disciplinary team in a community health setting.
The successful candidate will provide nursing and care management services to clients assigned to center prescribers, building relationships with internal and external medical providers to coordinate clients' access to services.
The ideal candidate will have a minimum of an Associate's Degree in Nursing or equivalent, and be licensed as a Registered Nurse (RN) by the RI Department of Health.
Prior experience in Psychiatric Nursing is preferred, as is experience in an electronic medical record (EMR). EBCAP provides training in NextGen, our EMR system.
