About This Role

This position offers a unique opportunity to work in a value-based care setting, where quality of care for patients takes precedence. As a Geriatric Care Physician, you will be part of an innovative team that prioritizes patient-centered care, spending time to listen, learn, and address factors affecting patient well-being.

Responsibilities
  • Evaluate and treat center patients according to standards of care.
  • Follow medical care and quality standards for patients, monitoring care using available data and chart reviews.
  • Assist in coordinating patient services, including specialty referrals, hospital and SNF coordination, durable medical equipment, and home health care.
  • Participate actively as a key source of medical expertise with the care team through daily huddles.
  • Help set a tone of cooperation in practice by displaying a professional demeanor.
Requirements
  • Current and unrestricted medical license or willingness to obtain a license in the state of practice.
  • Graduate of an accredited MD or DO program from an accredited university.
  • Excellent verbal and written communication skills.
  • Demonstrate high interpersonal relationship and communication skills with colleagues and patients.
  • Fully engaged in the concept of 'Integrated Team-Based Care' model.
  • Willingness and ability to learn and adapt to practice in a value-based care setting.
