Salary: Estimated annual salary range for this position is $80,000 - $110,000, depending on experience and qualifications.

Job Description: As a Patient Care Coordinator RN, you will play a crucial role in coordinating the continuum of care for assigned patients. Your primary responsibilities will include implementing evidence-based treatment guidelines, preparing patient charts prior to appointments, and following protocols for medication refills, test ordering, and routine preventative services.

Key Responsibilities:

  • Implement evidence-based treatment guidelines for clinically diagnosed conditions
  • Prepare patient charts prior to appointments
  • Follow protocols for medication refills, test ordering, and routine preventative services
  • Educate patients and families about managing their condition or disease
  • Conduct pre-visit planning with providers
  • Write individual care plans with treatment goals
  • Assess patient barriers to unsuccessful outcomes related to medication, appointments, and goals
  • Coordinate visits with multiple clinicians and/or diagnostic tests during one visit
  • Determine through triage how soon patients need to be seen
  • Provide telephonic advice on clinic issues
  • Identify health insurance or financial resources for patients
  • Systematically send or communicate medical information to patients
  • Review clinical measurements with patients at visits
  • Review information from facilities (discharge summary, hospital admission, or ER visits) to determine if patients require a follow-up visit or are at risk for adverse outcomes
  • Contact patients after discharge from facilities
  • Provide or coordinate follow-up care to patients/families who have been discharged
  • Develop written transition plans in collaboration with family and patients when transitioning to another facility or provider
  • Aid in identifying a new primary care physician or specialist/consultant and offer ongoing consultation

Requirements: To be successful in this role, you will need 3-5 years of experience in a clinic setting, with experience in specialty or primary care with EMR experience preferred. You must also possess excellent assessment skills, communication, written, and analytical skills.

Licenses and Certifications: A current license as a Registered Nurse in the state of Wisconsin is required upon hire.
