Overview
Kaiser Permanente, a leader in integrated health care, seeks an experienced Occupational Therapist to join our Home Health team. As a clinician with Kaiser Permanente, you will play a vital role in providing exceptional patient care and promoting optimal outcomes.
About the Role
This is a per hour position, with an estimated salary of $64-$90 per hour, depending on experience. The selected candidate will work as an independent practitioner, evaluating and treating patients in their homes. You will develop and implement individualized plans of care, focusing on functional abilities and adaptive equipment needs.
Responsibilities
- Evaluate patients to determine the nature and extent of dysfunction, considering adaptive equipment needs and Activities of Daily Living.
- Establish treatment goals and plan treatment to achieve established objectives, incorporating adaptive equipment as needed.
- Implement plans of care through direct patient treatment, delegating tasks as appropriate, and coordinating with other healthcare professionals.
- Assess and reassess patients for adaptive equipment needs, collaborating with certified vendors as necessary.
- Provide instruction and education to patients, families, and caregivers, promoting preventive and rehabilitative procedures.
- Regularly reassess clinical signs and symptoms to adjust treatment plans and goals.
- Evaluate the need for interdisciplinary referrals to RN, PT, ST, MSW, and HH Aide.
- Participate in multidisciplinary patient care conferences and discharge patients from occupational therapy when maximum benefit has been achieved.
- Maintain accurate and timely documentation of patient management, including adaptive equipment provision.
Qualifications
- Bachelor's or Master's degree in Occupational Therapy from an accredited program.
- Minimum 2 years of experience in occupational therapy, preferably in home health or related settings.
- California Occupational Therapist License and National Provider Identifier required at hire.
- Basic Life Support certification.
- Ability to detect early warning signs of medical complications and differentiate emergent versus non-emergent patient care status.
- Strong communication and interpersonal skills, with ability to work in a Labor/Management Partnership environment.
