Job Responsibilities- As a member of our nephrology healthcare team, you will participate in decision-making, teaching, leadership, and quality improvement activities that enhance patient care outcomes and home therapies program operations.
- You will identify and evaluate candidates for the Home Therapies Program, assessing their suitability and preparedness for dialysis treatments in the home setting.
- Perform comprehensive assessments and identify barriers to successful home dialysis, ensuring patients receive the necessary support and resources.
- Admit new patients to the Home Therapies Program, utilizing established protocols and procedures.
- Cordinate patient care, including assessment, planning, intervention, and evaluation for assigned hemodialysis patients.
- Analyze dialysis data and document adjustments or modifications to treatment plans as needed.
- Provide initial and ongoing education to patients and family members on home dialysis treatment and equipment operation.
- Evaluate patient awareness regarding transplant and treatment modality options.
- Administer medications as prescribed or in accordance with approved algorithms, documenting medical justification and effectiveness.
- Initiate or assist with emergency response measures, prioritizing patient safety and well-being.
- Graduate of an accredited School of Nursing.
- Current state licensure as a Registered Nurse.
- Successful completion of CPR BLS Certification.
- Meet practice requirements in the state where employed.
- Qualities and traits: compassion, caring, support for colleagues, collaborative, reliable.
- Experience as a Registered Nurse, preferably in chronic or home hemodialysis.
- Successfully pass the Ishihara Color Blind Test.
$85,000 - $115,000 per year, depending on experience and location.
