Job Responsibilities
  • As a member of our nephrology healthcare team, you will participate in decision-making, teaching, leadership, and quality improvement activities that enhance patient care outcomes and home therapies program operations.
  • You will identify and evaluate candidates for the Home Therapies Program, assessing their suitability and preparedness for dialysis treatments in the home setting.
  • Perform comprehensive assessments and identify barriers to successful home dialysis, ensuring patients receive the necessary support and resources.
  • Admit new patients to the Home Therapies Program, utilizing established protocols and procedures.
  • Cordinate patient care, including assessment, planning, intervention, and evaluation for assigned hemodialysis patients.
  • Analyze dialysis data and document adjustments or modifications to treatment plans as needed.
  • Provide initial and ongoing education to patients and family members on home dialysis treatment and equipment operation.
  • Evaluate patient awareness regarding transplant and treatment modality options.
  • Administer medications as prescribed or in accordance with approved algorithms, documenting medical justification and effectiveness.
  • Initiate or assist with emergency response measures, prioritizing patient safety and well-being.
Education and Qualifications
  • Graduate of an accredited School of Nursing.
  • Current state licensure as a Registered Nurse.
  • Successful completion of CPR BLS Certification.
  • Meet practice requirements in the state where employed.
Experience and Required Skills
  • Qualities and traits: compassion, caring, support for colleagues, collaborative, reliable.
  • Experience as a Registered Nurse, preferably in chronic or home hemodialysis.
  • Successfully pass the Ishihara Color Blind Test.
Estimated Salary Range:

$85,000 - $115,000 per year, depending on experience and location.
