Improve Lives as a House Calls Nurse Practitioner

We are seeking a compassionate and skilled Nurse Practitioner to join our team at Abode Care Partners. This role offers the opportunity to provide personalized medical care to seniors and individuals with complex needs in their homes or assisted living facilities.

About the Job

This is a full-time position that involves:

  • Planning, directing, and evaluating patient medical needs
  • Collaborating with healthcare professionals to ensure comprehensive care
  • Providing accurate medical documentation and ensuring patient records are up-to-date

The ideal candidate will have a strong understanding of gerontology and be able to communicate effectively with patients and their families.

Requirements
  • Active state NP license
  • Current NP National Board Certification
  • Active state DEA licensure
  • Master's degree in Nursing (MSN)
  • Five or more years of nursing experience; at least two years of NP experience preferred
  • Strong interpersonal communication skills – verbal and written
  • Excellent time management skills with accountability for practitioner's daily workflow
  • Proficiency in medical instruments and equipment
  • Strong knowledge base of HIPAA and OSHA regulations
  • Licensed driver with valid automobile insurance
  • Ability to engage in moderate physical activity, such as handling objects up to 25 lbs., standing or walking for more than four hours per day, etc.
What We Offer
  • Competitive annual salary range: $105,000 - $120,000 USD
  • Flexible schedule with weekdays and daytime shifts only
  • Mileage reimbursement for certain roles and delegated activities
