Home Health Nursing Opportunity

Key Responsibilities:

  • Perform comprehensive patient assessments and develop individualized plans of care in collaboration with the multidisciplinary care team.
  • Maintain continuity of patient care by communicating effectively with all team members and ensuring timely documentation.
  • Educate patients, caregivers, and families on aftercare and ongoing care.
  • Perform vital signs and patient assessments during visits.

Requirements:

  • Current and unencumbered license to practice as a Registered Nurse in the state of Maryland or eligible compact state licensure.
  • At least one year of Certified Home Health experience as a RN.
  • Current CPR certification using technical tools and software/EMR for visit documentation.
  • A reliable method of transportation, current driver's license, and car insurance coverage.
  • Previous experience completing home health OASIS visits.
  • Proof of Immunization/Exemption: Hep B, MMR, Varicella, Flu, and COVID-19.
  • TB Test or Chest X-Ray.
