About the Role

We are seeking an experienced Patient Care Coordinator to join our team at Excela Health. This is a dynamic role that requires strong communication and organizational skills, as well as the ability to work effectively with patients, families, and healthcare teams.

Responsibilities
  • Coordinate patient care throughout the continuum, from diagnosis to recovery, ensuring seamless transitions and effective communication among healthcare providers.
  • Develop and implement individualized care plans in collaboration with patients, families, and healthcare teams, focusing on enhancing patient outcomes and improving quality of life.
  • Serve as a single point of contact for patients and their families, providing education, support, and advocacy to ensure informed decision-making and optimal care.
  • Collaborate with multidisciplinary teams to coordinate services, including medical, nursing, ancillary, and community resources, ensuring comprehensive and coordinated care.
  • Develop and maintain relationships with healthcare providers, insurance companies, and community organizations to facilitate access to necessary services and resources.
  • Monitor patient progress, identifying opportunities for improvement and implementing changes to optimize care and outcomes.
Requirements
  • Bachelor's Degree in Nursing (BSN) or Master's Degree in Nursing (MSN) required.
  • Minimum 2 years direct medical/surgical experience, preferably in oncology or case management.
  • Strong leadership ability, independent thinking, and decision-making skills, with excellent oral and written communication skills.
  • Able to function in a team environment, with strong time management and organizational skills.
  • Ability to multitask, prioritize assignments, and demonstrate problem-solving skills, creativity, and flexibility.
Preferred Qualifications
  • Master's Degree in Nursing or related field (Health Administration, Business, Public Health, Management).
  • Working knowledge of databases and registries.
  • RN Case Management or care navigation experience in the last five years.
  • Three (3) years of oncology experience in a hospital, office, or ambulatory setting.
  • Knowledge of Payer/Insurance Benefits.
