We are seeking a highly skilled Director of Case Management to join our team at PAM Health, LLC. As a Director of Case Management, you will play a critical role in ensuring the highest levels of patient satisfaction and delivering exceptional patient care.

Key Responsibilities:
  • Coordinate the hospital-wide case management program to optimize financial and clinical outcomes.
  • Oversee the review of patient medical records to ensure proper utilization of hospital services.
  • Assist Case Managers in providing timely coordination of care and discharge planning.
  • Supervise Case Managers and oversee the daily operations of the case management department.
  • Analyze current systems and variance to identify opportunities for improvement and promote quality of care through collaboration with the interdisciplinary team.
Requirements:
  • Active RN License in the state where the hospital resides; BSN/MSN preferred.
  • Certification in an approved Case Management Program is preferred.
  • Current BLS certification required.
  • Three to five years of case management experience required.
  • Prior experience in IRF or LTACH setting preferred.
  • Prior management experience preferred.
