Compassionate Care Coordinator

About the RoleThis is an excellent opportunity for a skilled social worker to join our team and make a meaningful impact in the lives of our patients. You will be responsible for completing psycho/social assessments, developing care plans, and coordinating discharge transitions. Your strong interpersonal skills and knowledge of individual and family dynamics will enable you to provide emotional support and guidance to patients and their families during challenging times.
Responsibilities- Conduct thorough psycho/social assessments to identify patient needs and develop personalized care plans
- Coordinate with healthcare teams to ensure seamless transitions and effective communication
- Provide short-term adjustment counseling to address emotional, social, and economic stresses associated with illness and disability
To be successful in this role, you will need:
- A Master's degree in Social Work from an accredited institution and a Licensed Independent Clinical Social Work (LICSW) license
- At least 2 years of experience in an inpatient setting
- Strong organizational skills, ability to multitask, and excellent communication skills
- A working knowledge of community resources and systems
