Job Summary:
As a Licensed Practical Nurse at HRA, you will play a vital role in ensuring the quality and safety of healthcare services provided to our residents. Your primary responsibility will be to implement the plan of care formulated by the physician and the Director of Resident Care/Executive Director. This will involve providing accurate and timely documentation of the care given, assessing and providing resident and family/care giver information pertinent to diagnosis or plan of care, and administering medication per physician orders.
Key Responsibilities:
•Assess and Provide Resident Care: Conduct thorough assessments of residents to identify their healthcare needs and provide individualized care to meet those needs.
•Document Care Provided: Accurately and timely document all care provided to residents, including medication administration, treatments, and any changes in their condition.
•Collaborate with Healthcare Team: Work collaboratively with the healthcare team, including physicians, nurses, and other healthcare professionals, to ensure that residents receive comprehensive and coordinated care.
•Administer Medication: Administer medication to residents as ordered by the physician, ensuring that all medications are given in a safe and timely manner.
•Maintain Resident Records: Maintain accurate and up-to-date records of resident care, including progress notes, medication lists, and other relevant information.
Requirements:
•Licensed Practical Nurse (LPN) License: Must be currently licensed as a Licensed Practical Nurse in the state of practice.
•Experience: Minimum one year of experience as a Licensed Practical Nurse, preferably in a long-term care or geriatric setting.
•Skills: Strong assessment and documentation skills, with the ability to work collaboratively as part of a healthcare team.
