Salary: $65,000 - $85,000 per year, depending on experience.
About the Role
We're seeking a detail-oriented and organized professional to assist the Care Manager Social Worker in supporting patients and families in the hospital or ambulatory setting related to psychosocial implications of illness and transitions of care.
Key Responsibilities
- Demonstrate expertise in patient assessment, treatment planning, and communication skills tailored to diverse patient populations.
- Utilize electronic medical records and referral systems to access relevant patient information.
- Collaborate with interdisciplinary teams to develop and implement comprehensive care plans.
- Assist with initial assessments, referrals to post-acute providers, and insurance authorizations.
- Support special projects such as updating resource information and issuing regulatory notices.
- Foster teamwork and flexibility in a fast-paced environment.
Requirements
- Bachelor's Degree and enrollment in an accredited MSW/Master of Social Work program preferred.
- BSW without MSW program enrollment required.
- Strong organizational and communication skills.
- Ability to work effectively in a team environment.
