Hospice Liaison

About the Role: Amedisys, a leading home health and hospice company, is seeking a Hospice Liaison to join their team. This role involves educating healthcare professionals, patients, and families about home care services, developing relationships with key referral sources, and coordinating patient admissions.

Responsibilities:

• Educate healthcare professionals, patients, and families about home care services.
• Develop and maintain relationships with key referral sources, such as hospitals, nursing homes, and physician offices.
• Coordinate patient admissions with the team, including obtaining required documentation.
• Conduct on-site visits to assess patient needs and identify barriers to care.
• Keep accurate records of patient interactions and referrals.
• Achieve admissions, product, and payor-mix goals.
• Meet short and long-term target account goals.
• Document sales calls and maintain records of targeted referral sources.
• Attend community events to promote Amedisys.
• Respond to customer complaints in a timely manner.

Requirements:

• At least one year of experience in a healthcare-related business development or outreach role.
• Reliable transportation, current driver's license, and liability insurance.
