Job Summary:

Certified Diabetes Educator or certification eligible RN, RD, Pharm D, or Social Worker employed by Medicine Journal is responsible for providing direct and indirect care to patients under the supervision of a Physician or Nurse Practitioner/Physician Assistant. This role involves a team-based approach to care, with team members trained to meet the highest level of function as per the State of Tennessee/Georgia guidelines.

The Diabetes Educator will be trained and assigned to coordinate care for patients and support patients/families/caregivers in self-management, self-efficacy, and behavior change. This role also involves managing the practice's patient population, participating in the practice's quality improvement process, and performance evaluation.

Education:
Required:
Graduate from an approved program in dietetics/nutrition or a Registered Nurse or Certified Diabetes Educator. CDE eligible Pharm D or Social Worker.

Experience:
Required:
A minimum of three years of progressive experience in a hospital or clinic setting required. Experience in Insulin Pump Education.

Position Requirement(s):
Required:
Current professional registration required. BLS prior to going to the unit/department and maintained going forward.

Department Position Summary:
Establishes and coordinates the diabetes self-management training for patients with diabetes. The majority of patients seen are adults. Participates in client-centered case management and coordinates multidisciplinary plan of care.

The employee must be able to demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served in the clinic. The individual must possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his or her age-specific needs and to provide the care needed as described in the department's policies and procedures.

The employee must be able to demonstrate the knowledge and skills needed to provide education to adult learners. Teaches diabetes self-management to individuals and classes. Helps manage outcomes. Assist with other activities that promote and encourage diabetes education as assigned.
