We are seeking a skilled Health Coach to join our team at PSG Global Solutions. As a Social Work Health Coach in Primary Care, you will play a vital role in supporting patients with Attention-Deficit/Hyperactivity Disorder (ADHD).
Key Responsibilities- Collaborate with primary care teams to provide comprehensive care to patients with ADHD.
- Develop and implement personalized health coaching plans to promote patient engagement and self-management.
- Conduct thorough assessments and create tailored interventions to address patients' unique needs.
- Work closely with healthcare providers to ensure seamless care coordination and communication.
- Stay up-to-date with the latest research and best practices in ADHD care and health coaching.
- Master's degree in Social Work or a related field.
- Experience working in primary care or a related setting.
- Strong knowledge of ADHD and its impact on patients and families.
- Excellent communication and interpersonal skills.
- Ability to work effectively in a team environment.
