Memory Care Support Specialist

Starting Pay Rate: $20/hour

The Memory Care Support Specialist plays a vital role in our mission by assisting residents with personal care tasks, including dressing, grooming, personal hygiene, and mobility, all aimed at promoting independence in accordance with individualized service plans and established protocols.

Key Responsibilities:

  • Assist or oversee residents with personal care to foster the highest level of independence.
  • Monitor and communicate changes in resident conditions to supervisors and/or the Assistant Director of Nursing.
  • Ensure accurate and timely documentation for individual service plans, medication logs, and activity records.
  • Administer medications as per physician orders or supervise self-administration.
  • Support daily housekeeping tasks to maintain a clean and safe environment, including sorting laundry as necessary.
  • Assist with meal preparation, serving, and the cleaning of kitchen and dining areas.
  • Plan and facilitate leisure activities as outlined in individualized service plans.

Qualifications:

  • High School Diploma or G.E.D.
  • Community Based Residential Facility (CBRF) certification is preferred but can be provided.
  • Fire safety training is preferred but can be provided.
  • CNA certification is preferred.
  • American Heart Association (AHA) First Aid certification must be obtained within 6 months of hire or within 2 years of initial CBRF First Aid & Choking certification class. AHA First Aid certification is valid for 2 years and must be renewed thereafter.
  • Must become medication delegated upon hire and be capable of administering medications.
  • Experience in a resident care role is preferred.
  • Able to work independently and make decisions within established guidelines.
  • Capable of lifting and moving objects weighing up to 60 pounds frequently.
