Salary: We offer a competitive salary range of $85,000 - $110,000 per year, depending on experience and qualifications.

Job Description: As a Registered Nurse (RN) Case Manager with our Hospice Services team, you will play a vital role in delivering patient-centered care to individuals facing life-limiting illnesses. Your primary responsibilities will include:

  • Conducting comprehensive evaluations and assessments to identify individualized care needs">
  • Developing and implementing personalized care plans that promote comfort, dignity, and quality of life">
  • Maintaining accurate and timely documentation and records to ensure seamless continuity of care">

Required Skills and Qualifications: To be successful in this role, you must possess:

  • A current, active, and unrestricted Registered Nurse (RN) licensure in the state of practice">
  • Strong assessment, planning, and problem-solving skills">
  • Excellent communication and interpersonal skills">
  • The ability to work independently and as part of a collaborative team">
