Independent Optometrist

As an Independent Optometrist leasing space inside a Sam's Club, you will play a crucial role in providing patient care for eye and vision health, as well as general well-being. This position offers autonomy and flexibility to manage your practice, including setting your own schedule, hours of operation, patient schedule, and fee structure.

• Establish and manage your patient schedule
• Control your fee schedule and keep 100% of exam/professional fees
• Option to participate in equipment/instrumentation and administrative/Opt-Tech support services for a fair market value

• Doctor of Optometry (OD) degree from an accredited optometry school
• Valid state optometry license
• Ophthalmologist (MD or DO) degree from an accredited school (for certain locations)
• Valid state ophthalmology license (for certain locations)
• General Liability Certificate
• Professional Liability Insurance Certificate
• W-9 form
