Job Overview

We are seeking Support Coordinators to assist our team in providing individualized support and services to our clients. As a Support Coordinator, you will work directly with clients to understand their needs and preferences, and develop personalized plans to help them achieve their goals.

Responsibilities

Your primary responsibilities as a Support Coordinator will include:

  • Conducting initial assessments and developing individualized service plans;
  • Providing direct support and assistance to clients with daily living tasks, such as personal care, meal preparation, and transportation;
  • Assisting clients with behavioral or medical complexities;
  • Documenting client progress and maintaining accurate records;
Requirements

To be successful in this role, you will need:

  • A high school diploma or equivalent;
  • 1-2 years of experience working with individuals with disabilities;
  • Excellent communication and interpersonal skills;
  • Ability to work independently and as part of a team;
  • Basic computer skills and proficiency in Microsoft Office applications;
