Job Description

Summary:

The Senior Care Nurse Practitioner will identify and address health needs of seniors within the senior care program. This position is in person, with frequent local travel required. Hours are Monday-Friday, 8a-4p.


Responsibilities:

  • Conduct assessments to identify normal aging processes and how to address those in the care plan.
  • Collaborate with the interdisciplinary team to ensure proper care for conditions found in aging adults.
  • Assist patients in determining end-of-life care decisions and advanced directives.
  • Gather information from patients and families to help create a better picture of at home life and any conditions affecting patients.


Requirements:

  • Current license in Massachusetts as a Nurse Practitioner.
  • Elder experience preferred
  • Current CPR Certification
  • Frequent local travel with access to personal vehicle
  • Covid and Flu Vaccinations required

Salary: $100,000-$120,000/yr based on experience
